For your convenience we accept cash, personal checks, Visa, and MasterCard. We deliver the finest care at the most reasonable cost to our patients, therefore, payment is due at the time service is rendered unless other arrangements have been made in advance. Patient Responsibility (without insurance):

Payment in full is required on the date of service.

Patient Responsibility (with insurance):

We are happy to assist you by verifying your insurance benefits and obtaining pre-authorization for treatment. If you are eligible for insurance coverage, you will be required to pay approximately one third or 30% of your estimated fee on the day of surgery. This is the estimated deductible and co-pay not covered by your insurance. Upon receiving your insurance payment, we will send you a statement for the remaining balance or in the event of an overpayment, the appropriate refund. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. Please remember, you are fully responsible for all fees charged by this office, regardless of your insurance coverage.

Payment Plans

For those procedures that do not qualify for insurance reimbursement, we can assist you in tailoring a payment plan that will fit your needs. Plans typically allow up to 60 months for repayment and are administered by specialized health care finance companies, such as, Capital One and American General. Participation in these plans is subject to qualification and must be approved prior to your treatment.
